With the gradual resurgence of low-carb diets in recent years, the word “ketones” is thrown around a lot. Many people aren’t really aware of the details. So just what are ketones?

Ketone bodies are three water-soluble molecules (acetoacetate, beta-hydroxybutyrate, and their spontaneous breakdown product, acetone) containing the ketone group that are produced by the liver from fatty acids[1] during periods of low food intake (fasting), carbohydrate restrictive diets, starvation, prolonged intense exercise,[2] alcoholism or in untreated (or inadequately treated) type 1 diabetes mellitus. These ketone bodies are readily picked up by the extra-hepatic tissues (tissues outside the liver) and converted into acetyl-CoA which then enters the citric acid cycle and is oxidized in the mitochondria for energy.[3] In the brain, ketone bodies are also used to make acetyl-CoA into long-chain fatty acids.

Ketones are formed when there is not enough sugar or glucose to supply the body’s fuel needs. During these periods, insulin levels are low, but glucagon and epinephrine levels are relatively normal. This combination of low insulin, and relatively normal glucagon and epinephrine levels causes fat to be released from the fat cells. Blood sugar levels never get too high, because the production is regulated by just the right balance of insulin, glucagon and other hormones.

Exogenous ketones (also known as ketone supplements) and well-formulated ketogenic diets share at least one thing in common. As the pace quickened during a mountain stage of the 2013 Tour de France, a jolt of excitement rippled through the peloton: one of the leaders had fumbled the feed bag containing his next drink. “I’d never, ever seen so much chatter on the radio,” Greg Henderson, a veteran bike racer from New Zealand, later told an interviewer.

Eventually, a team car stopped to collect the bag from the side of the road where it had fallen, and another rider shuttled it back up to the cyclist.

The answer, he now suspects, was ketones, the much hyped “fourth macronutrient” that offers a sustained—and legal—source of energy for muscles beyond the traditional carb­ohydrates, fats, and proteins. For several years, whispers of an astronomically ex­pensive, ketone-based superdrink have percolated through the elite sports world, par­tic­ularly among cyclists.

Ketosis is a metabolic state in which fat becomes the body’s primary fuel source instead of usual its go-to: blood sugar (aka glucose) which is derived from carbs you eat.

When you follow a high-fat, moderate protein, low-carb diet, glucose levels remain steady, and insulin doesn’t surge. Instead, dietary or body fat is broken down into organic compounds known as ketones.

“If there’s a downside, it is kind of crazy tasting,” said Geoff Woo, the founder of HVMN, a Silicon Valley company that makes nootropics, or performance-enhancing supplements.

It was a small, clear vial labeled “Ketone,” a new type of energy drink his company is releasing this week. About 25 minutes after I drank Ketone, Woo and Stubbs pricked my finger to see if it was working. My blood sugar, which had verged on diabetic levels from some pineapple I had eaten that morning, was down to near-normal levels.

The process of burning these ketones is called “ketosis,” and it can be achieved through either fasting or through a “ketogenic” diet that’s high in fat and very low in carbohydrates. Today, some healthy people fast intermittently or follow ketogenic diets in order to remain in a near-constant state of ketosis, saying it helps them control their weight, feel more energetic, and stay focused.

One miraculous energy source has garnered the attention of many nutrition experts and researchers over the past decade. We call them ketones or ketone bodies (more on the difference between the two later).

Glucose is the primary energy source for almost every cell in the body. This is because it can be broken down into energy much more quickly than any other fuel source, and it does this without the help of the mitochondria (the main energy producing component of the cell).

Ketones are a more efficient fuel source that inhibits the production of free radicals and reactive oxygen species. For example, studies done on people with different types of cognitive issues from Parkinson’s disease to epilepsy confirm that using ketones as fuel can improve brain function tremendously.

Elevating blood ketones is not a good idea for everyone. “Diabetic” because this happens in people with type 1 and type 2 diabetes, and “ketoacidosis” because the excessive amount of ketones causes the blood to become too acidic.

The most common symptoms are thirst, frequent urination, nausea, abdominal pain, weakness, fruity-scented breath, and confusion. If you have these symptoms, drink plenty of water and they may subside. Consult with your doctor if symptoms don’t get any better.

When your body uses energy from ketones, you are said to be in a metabolic state called ketosis. This means, on keto, you can eat as much delicious coconut oil, butter, and eggs as you want.

Only a small number of ketones are manufactured on a large scale in industry. They can be synthesized by a wide variety of methods, and because of their ease of preparation, relative stability, and high reactivity, they are nearly ideal chemical intermediates. Many complex organic compounds are synthesized using ketones as building blocks.

High in saturated fat and VERY low in carbohydrates, “keto” is adopted by a growing population to paradoxically promote weight loss and mental well-being. To the uninitiated, keto defies all common sense, inviting skeptics to wave it off as an unnatural “bacon-and-steak” fad diet.

Emerging evidence from animal models and clinical trials suggest keto may be therapeutically used in many other neurological disorders, including head ache, neurodegenerative diseases, sleep disorders, bipolar disorder, autism and brain cancer.

The backup is ketone bodies that the liver derives primarily from fatty acids in your diet or body fat.
